Hormone Discovery Call – Treatment Plan

The Hormone Discovery Call – Treatment Plan is a 1-hour consultation designed for individuals experiencing thyroid, adrenal, or sex hormone imbalances, including symptoms related to hypothyroidism, perimenopause, menopause, PCOS, adrenal fatigue, low testosterone, or post-birth control recovery. This call is the first step toward being considered for the Hormone Treatment Plan at Empower Functional Health.

The Hormone Treatment Plan (valued at $3,675) includes one Personalized Health Plan (PHP), six 1-on-1 practitioner sessions, access to 2–3 prescription medications, and three months of access to our practitioner-led Chronic Illness/CIRS Group.

What this includes

• 1-hour functional health consultation with an EFH practitioner trained in hormonal restoration and functional medicine
• Review of up to 2 prior labs or tests, plus health, diet, and medication history
• Exploration of symptoms and goals related to thyroid, adrenals, sex hormones, and overall metabolic function
• Discussion of our hormone restoration approach, including how diet, lifestyle, sleep, and stress balance impact hormonal health
• Written session notes summarizing insights, personalized recommendations, and next steps

How it works

After purchase, you’ll receive intake forms and scheduling instructions within 2 business days. Once submitted, you’ll be matched with an EFH practitioner experienced in hormone, thyroid, and adrenal recovery. Your practitioner will review your forms prior to your consultation to ensure a
comprehensive and efficient discussion.

• This call is required for those considering the Hormone Treatment Plan, but does not guarantee acceptance. We must first ensure your case is a good fit for our model of care.
• The cost of the Discovery Call is separate from any lab testing, prescriptions, or treatment plan fees.
• The session is conducted by a qualified EFH practitioner, not Judy Cho, though all practitioners are trained in functional medicine and hormone restoration.
• Group access, additional testing, and supplements are not included in this call but may be recommended afterward.
